First Visit

Thank you for choosing HorizonView Health for your medical needs. To better serve you, please arrive 15 minutes early to your appointment. If you are unable to complete registration forms or additional paperwork prior to your appointment, please arrive 30 minutes early. Please bring your insurance card, photo ID, and any applicable co-pays.

Our physicians and clinical staff are experienced in the latest advancements in healthcare. Our office believes that good medicine begins with open communication. We promise to speak openly with you about symptoms, diagnosis, and recommendations. We highly encourage patients to ask questions during their visits and to utilize the patient portal for any questions that may come up after an appointment.

No Show/Cancellation Policy

We understand that sometimes circumstances beyond your control may result in canceling a scheduled appointment. Therefore, if you are unable to keep your appointment, we kindly ask that you notify us 24 hours in advance of your scheduled visit. Failure to do so may result in a $35 no‑show fee. Behavioral Health patients may be subject to a $75 no show fee.

Unless other arrangements have been made, all payments are due at time of service. We accept the following forms of payment:

Prescription Refills

  • If you require a written prescription, please leave a message on our refill request line with the requested information. Please allow 24-48 business hours for your request to be processed.
  • For routine refills, please contact your pharmacy. They will fax us with the necessary information to process the request. Please allow 24-48 business hours for your request to be processed.
